Description
This is a full‑time 1:1 position in Essex, Ia. The hours are Monday – Friday 6 am – 2 pm.
I. The Supported Community Living Specialist (SCLS) reports directly to the Team Leader assigned and to the Director of Residential Services.
Responsibilities- The SCLS will teach, train, and assist individuals in developing the skills necessary to be as independent as possible when performing activities of daily living, including but not limited to:
- Coordinating health and medical needs.
- Training individuals to take care of their own health and medical needs.
- Developing or maintaining skills that allow for better participation in the community.
- Assisting individuals in becoming independent in accessing community services.
- Assisting the individuals with participation in at least one community activity each week if the shift is during day/evening/weekend hours.
- Assisting individuals in advocacy procedures to ensure their rights and to meet their needs, interests, and choices.
- Assisting in preparing an individualized diet plan to meet nutritional needs as required.
- Writing individualized intervention plans for each individual served.
- Providing emergency/safety skills training.
- Providing training in skills necessary to manage personal business and property.
- Providing training in budgeting.
- Providing transportation (doctor’s appointments, church, etc.).
- Providing training in daily living skills.
- Orienting new individuals in their home, assessing their skills, and providing appropriate input to the individual’s subsequent progress reports and Person‑Centered Plan (PCP).
- Supervising the individual’s goals and completing data collection related to running those goals.
- Supervising, as necessary, the individual’s finances to give guidance in budgeting for personal needs and desires.
- Providing a supportive and therapeutic atmosphere.
- Supervising the individual’s care and use of personal property and assisting with purchases, as necessary.
- As assigned, training new staff relative to duties, work schedules, agency policies, and state and/or federal regulatory policies and procedures in coordination with the Team Leader and Director of Residential Services.
- Following the assigned work schedule.
- Never leaving individuals receiving twenty‑four‑hour care unsupervised.
- Completing all records and documentation using the proper forms as they relate to established procedures.
- Attending staff meetings and scheduled in‑service training sessions.
- Ensuring that the individual’s residence meets cleanliness, safety, and health standards.
- Keeping informed of all programming and administrative information, changes, and additions by checking email each shift worked the daily logbook or other messages.
- Keeping all written and verbal information on each individual strictly confidential.
- Completing documentation of behaviors or providing information for writing staff intervention plans.
- Handling any other duties as assigned by the Team Leader, Lead Staff, and Director of Residential Services.
- Monitoring and documenting an individual’s physician‑ordered diet as needed.
- Assisting individuals receiving services with their activities of daily living, such as, but not limited to, meal preparation, laundry, housekeeping, toileting, and bathing.
- Ensuring the clients are safe from all environmental hazards.
- Providing quality care.
- Transporting individuals.
- Providing training in daily living skills (e.g., meal preparation, laundry, cleaning, and personal hygiene).
- Ensuring housekeeping standards are maintained.
- Workers must be willing to work with individuals who have behavioral problems, including verbal and physical aggression.
- Providing a supportive and therapeutic atmosphere.
- Administering medication and maintaining accurate medication documentation, as needed, after training.
- Using sound judgment when immediate decision‑making is required.
- Working according to the assigned work schedule; overnight staff may remain awake or sleep while remaining available for emergencies.
