Description:

Social Work Practicum Intern

The Social Work Practicum Intern is a temporary educational position designed to provide observational learning experiences within Trivium Life Services programs. The Practicum Intern will shadow assigned staff including, but not limited to, Residential DSPs, LINKS Coaches, Service Coordinators, and Residential Team Coordinators to gain exposure to service delivery, person-centered planning, behavioral supports, regulatory compliance, and interdisciplinary collaboration.

Essential Duties

This position includes, but is not limited to, the following essential functions:

  • Observe day-to-day service delivery in residential, community-based, and day habilitation settings.
  • Observe implementation of Person-Centered Plans, behavioral support plans, and therapeutic milieu practices.
  • Shadow staff during team meetings, interdisciplinary meetings, and internal coordination meetings when appropriate and authorized.
  • Observe documentation processes including progress notes, incident reporting, ISP data entry, and regulatory compliance practices.
  • Observe coordination of services related to medication management, appointments, housing supports, and benefit coordination.
  • Maintain professional conduct and represent Trivium Life Services in a positive and ethical manner.
  • Treat persons served with dignity and respect at all times.
  • Maintain strict confidentiality of all client, employee, and organizational information.
  • Participate in scheduled supervision meetings with assigned supervisor to review learning objectives, progress, and end of practicum evaluation.
  • Complete any required school documentation, reflection assignments, or evaluation materials as required by their academic institution.
  • Attend required orientation and compliance training prior to beginning shadow experiences.
  • Follow all Trivium Life Services policies, procedures, Corporate Compliance policies, and Code of Ethics.
  • Other duties as assigned that are appropriate to an observational educational role.
Practicum Intern Not
  • Provide direct care or supervision of persons served.
  • Administer medications.
  • Transport persons served.
  • Access electronic health records beyond approved observational access.
  • Independently implement behavioral or service interventions.
