Physical Therapist; PT

Physical Therapist – Clarion Forest VNA is looking to hire a full, part‑time, or per diem PT to provide therapy services to our patients in their homes primarily in Clarion and Forest Counties.

Full‑time PTs are salaried with a bonus system in place. Starting salary ranges between $68,000–$85,000 per year for full‑time. Salary commensurate with education and experience.

• Conduct thorough assessments to evaluate patients’ physical conditions and develop personalized treatment plans.
• Provide skilled physical therapy services to patients in their homes, focusing on improving mobility, strength, and function.
• Educate patients and their families on exercises and techniques to enhance recovery and prevent further injury.
• Collaborate with an interdisciplinary team to ensure comprehensive and coordinated patient care.
• Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
• Maintain accurate and timely documentation of patient care and progress.

• Master’s or Doctorate degree in Physical Therapy from an accredited program.
• Licensed Physical Therapist in Pennsylvania

• Previous experience in home health or hospice care is preferred, but not required.
• CPR certification.
• Current valid PA Driver’s License and reliable transportation to make home visits.
• Strong communication and interpersonal skills.
• Ability to work independently and manage a flexible schedule.
