Position Overview
Aveanna Healthcare has grown during the past year through a series of exciting acquisitions and is looking for a Physical Therapist (PT) to join our dynamic home‑health care team. In this full‑time, Monday–Friday role located in Lake County, Tavares, and Eustis, you will help patients recuperate from illness or injury in the comfort of their own homes.
Essential Job Functions- Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function.
- Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes.
- Provide a written evaluation and plan of care defining physical therapy needs, goals, progress, and expected outcomes.
- Deliver physical therapy services in compliance with prescribed physician orders.
- Maintain plan of care by performing timely documentation of client performance, modifying goals and treatment as needed, and determining discharge plans.
- Plan and educate patients and caretakers on home exercise programs post‑discharge.
- Coordinate services with the healthcare team including physicians, nurses, parents/guardians, etc.
- Document according to policy and procedure with minimal supervision, ensuring no technical or clinical denials after payer review.
- Maintain effective communication with staff, healthcare team members, and families.
- Serve as a resource consultant for clinical staff.
- Supervise other personnel (PTA, OTA, HHA) as applicable.
- Complete OASIS assessments where allowed by state regulation.
- Follow policies and procedures and conform to all applicable patient care standards, licensure regulations, and state laws.

- Degree in Physical Therapy from an accredited university/program.
- Current, unrestricted state license as a Physical Therapist in the state of practice.
- Valid CPR and driver’s license.
- Experience in Home Health Therapy.
- Completion of OASIS documentation.
