Innovate Rehab and Wellness, in partnership with Hillcrest Health & Living, is looking for a collaborative and clinically strong Occupational Therapist Rehab Program Coordinator to lead and elevate therapy services within our senior care communities. This is an opportunity to step into a leadership role where you will shape clinical outcomes, mentor a multidisciplinary therapy team, and drive excellence in rehabilitation services for aging adults.

What You’ll Do

As the Occupational Therapist Rehab Program Coordinator, you will serve as the clinical and operational leader of the therapy program. You will support high‑quality, efficient, compliant rehabilitation services while partnering with nursing, facility leadership, patients, families, and the interdisciplinary team.

You Will
  • Lead and coordinate an interdisciplinary therapy team, including Physical Therapy, Occupational Therapy, and Speech Therapy.
  • Support strong patient outcomes through effective care planning, clinical oversight, communication, and collaboration.
  • Oversee therapy scheduling, caseload management, team productivity, and daily operational needs.
  • Manage and monitor the PPS process, including accurate coding, documentation, care planning, and regulatory compliance.
  • Ensure therapy practices align with federal and state regulations, reimbursement requirements, and professional standards.
  • Review therapy plans of care, interventions, progress, outcomes, documentation, and fiscal performance.
  • Lead quality improvement efforts through QAPI initiatives, audits, performance monitoring, and action planning.
  • Partner with nursing and facility leadership to support long‑term care programming and coordinated patient care.
  • Participate in triple‑check processes, annual budget planning, and resource management.
  • Support onboarding, mentoring, coaching, and professional development for therapy team members.
  • Provide direct patient care within your licensed discipline as needed.
  • Ensure therapy equipment, supplies, work spaces, and treatment environments meet safety and operational standards.
  • Build positive, professional relationships with patients, families, providers, team members, and community leaders.
What We’re Looking For

We are looking for a therapist who is ready to grow into or continue in a leadership role while remaining connected to patient care. The right candidate will be organized, clinically confident, and able to balance team leadership, compliance, quality outcomes, and daily operational priorities.

Ideal Candidates Will Have
  • One to two years of therapy experience in acute care, post‑acute care, skilled nursing, rehabilitation, or a similar setting.
  • Prior leadership, team lead, supervisory, or operational management experience preferred.
  • Strong understanding of therapy documentation, care planning, clinical outcomes, and reimbursement processes.
  • Ability to lead, coach, and support a multidisciplinary therapy team.
  • Strong communication, organization, problem‑solving, and decision‑making skills.
  • Ability to manage multiple priorities in a fast‑paced environment.
  • A calm, flexible, and professional approach when working with patients, families, and team members.
  • Comfort using healthcare technology, documentation systems, and productivity tools.
  • A commitment to compassionate, patient‑centered care and continuous improvement.
Education & Credentials
  • Graduate of an accredited Occupational Therapy program required.
  • Active Occupational Therapist license in Nebraska or Iowa, based on assigned location, required.
  • Additional licensure, certifications, or credentials may be required based on community and regulatory standards.
