Location: Chariton
Elara Caring is seeking a Medical Social Worker (MSW) to join our home care team. The MSW will assess psychosocial and environmental needs, develop and revise care plans, and collaborate with physicians and families to ensure safe, quality care in the patient’s home.
This PRN role requires 50% travel and a valid Social Work license. The successful candidate will have a Master’s (or Doctoral) in Social Work, 1+ year healthcare social work experience, and strong communication skills.
