Obtain and record residents' vital signs and report deviations to supervising nurse;
Assist residents with personal care including bathing, grooming, dressing, toileting, incontinence care, nutrition, ambulation, transfers, positioning, and medication reminders;
Follow individualized service plans and daily task sheets and communicate status changes to supervisors;
Maintain safe and clean resident environments and document progress;
Uphold HIPAA confidentiality and participate in mandatory trainings/biweekly in-service meetings
