Social Services Supported Community Living Specialist

Job Overview

This part‑time position (30 hours per week on average) is located in Red Oak, Iowa. Hours are Monday/ Tuesday 3 pm–11 pm, and every other weekend 9 am–11 pm. The Supported Community Living Specialist (SCLS) reports directly to the assigned Team Leader and to the Director of Residential Services.

• Teach, train and assist individuals in developing skills for independent activities of daily living, including but not limited to health and medical self‑management, community participation, and day‑to‑day living skills.
• Coordinate health and medical needs, train individuals to manage their own health, and assist with advocacy procedures to ensure rights and needs are met.
• Write Individualized Intervention Plans and support the development of Person‑Centered Plans (PCP).
• Provide emergency/safety skills training and train individuals to manage personal business, property, and budgeting.
• Provide transportation to doctor’s appointments, church, and other activities.
• Assist individuals with participation in at least one community activity each week when shifts occur during day, evening or weekend hours.
• Supervise and record progress toward individuals’ goals and collect data related to those goals.
• Provide a supportive and therapeutic atmosphere; ensure safety, cleanliness, and health standards are met at all times.
• Never leave individuals who require twenty‑four‑hour care unsupervised.
• Complete all required documentation in accordance with agency procedures, maintain confidentiality, and attend staff meetings and in‑service training sessions.
• Assist with daily living activities such as meal preparation, laundry, housekeeping, toileting, bathing, and transporting individuals.
• Assist with cleaning of biological fluids and other waste as necessary.
• Follow the assigned work schedule and remain available for emergencies during overnight or long shifts.
• Monitor and document an individual’s physician‑ordered diet as needed.
• Maintain PHI security clearance appropriate for the SCLS role (Level Two), comply with HIPAA, and keep all information confidential.
• Other duties as assigned by the Team Leader, Lead Staff or Director of Residential Services.

• Provide quality care and transportation to individuals.
• Administer medication and maintain accurate medication documentation after training.
• Train and supervise new staff relative to duties, work schedules, agency policies, and state/federal regulatory policies in coordination with the Team Leader and Director of Residential Services.
• Work according to the assigned schedule; allow sleeping during overnight shifts if the staff remains reachable for emergencies.
• Execute sound judgment and make immediate decisions when required.
• Comply with state and federal regulations regarding HIPAA and confidential PHI handling.
• Work independently to ensure the welfare and safety of persons served in a residential setting.
• Ensure housekeeping standards are maintained and that the residence meets cleanliness, safety, and health standards.
• Complete written and electronic documentation and record keeping according to agency requirements.
• Assist with monitoring physician‑ordered diet, and document behavior to guide staff intervention plans.
• Maintain security clearance levels for PHI access as outlined for the SCLS role.

• Must be 18 years of age or older.
• High school diploma or GED required; preferred experience teaching cooking, cleaning, and personal hygiene skills.
• Valid Iowa chauffeur’s license with an acceptable driving record.
• Successful completion of pre‑employment physical, TB test, drug screening, and compliance with subsequent medical schedule requirements.
• Complete 10‑hour medication‑management class.
• Willingness to work with individuals who have behavioral disorders and may display aggressive outbursts.
• Willingness to clean any biological fluids or waste.
• Strong verbal and written communication skills, with the ability to work as part of a team.
• Self‑motivated and capable of exercising sound judgment, especially in emergencies.
• No legal convictions related to assault, abuse, or bodily injury.
• Participation in agency investigations as required; confidentiality will be respected.

• Travel up to 70 miles from work location.
• Enter and exit resident homes that may present accessibility challenges.
• Walk up and down stairs while carrying items such as laundry.
• Work full scheduled shifts as assigned.
• Carry supplies up to 50 pounds as needed.
• Provide physical assistance for transfers and personal care of individuals.
• Assist wheelchair users with transfers to chairs, beds, toilets, and baths.
• Assist individuals in shower or bath settings.
• Transport household items and groceries between vehicle and home multiple times.
• Perform snow removal as necessary for individual and staff safety.
