The purpose of a Clinical Informatics Specialist (CIS or Medical Scribe) at Oak Street Health is to support our primary care providers with clinical documentation so they can focus on providing exceptional patient care. Scribes assist providers throughout the patient care journey – from morning huddles to exam room observation, and post‑visit follow‑up. They become experts in our value‑based care model, chronic condition documentation and coding, helping providers identify and close care gaps.

Scribes receive extensive on‑the‑job training in clinical workflows, preventative care, documentation accuracy, and population health data.

• Document patient encounters (~80%) – join the provider in the exam room, observe visits, record history, assessment, plan, and physical exam, assign CPT and ICD‑10 codes, prepare after visit summaries, and consult with the provider to ensure accurate and specific documentation.
• Clinical Documentation Improvement (~10%) – request and review medical records, leverage Oak Street’s population health tools to support documentation improvement, attend daily huddles and documentation reviews, and consult with providers on documentation opportunities.
• Administrative support (~10%) – place orders and referrals, address tasks, support the care team with additional documentation responsibilities, and perform other duties as assigned.

Knowledge:

• Medical terminology and common medications – required knowledge from pre‑medical or prior clinical experience.
• Prior clinical experience, including shadowing or volunteering – strongly preferred.
• Prior scribe or transcription experience – preferred but not required.

Skills:

• Advanced listening and communication skills – required.
• Strong computer literacy and ability to learn new technical workflows – required.
• Fluency in Spanish, Polish, Russian or other community languages – required where indicated.

Abilities:

• Adapt to new workflows and quickly learn new concepts – required.
• Type 70+ words per minute – strongly preferred.
• Take direction and work as a team member – required.
• Self‑starter within role scope – required.
• Excellent attendance and willingness to work in-person at clinics – required.
• Commit to at least 1 year in the role (2+ years ideal) – required.
• Work approximately 40–45 hours per week during clinic hours – required.
• Comply with hospital and Oak Street Health policies, including HIPAA – required.
• U.S. work authorization – required.

Pay Range: $17.00 – $31.30 per hour

Position eligible for a CVS Health bonus, commission, or short‑term incentive program in addition to the base pay range.
