Physical Therapist (PT) - Must Be Licensed

Job Overview

A great SNF / LTC seeks an experienced or newly graduated Physical Therapist / PT for a contract assignment.

• Assess referred patients requiring rehabilitation services, including direct patient care to evaluate medical condition, functional capabilities, limitations, restrictions, and potential for rehabilitation.
• Establish and administer treatment programs with specific goals determined by the patient’s capacity and tolerance under physician direction.
• Set realistic, achievable goals; document and record patient condition; educate patients and families on appropriate physical therapy methods.
• Develop a plan of care for each patient and provide skilled physical therapy services/interventions per physician orders.
• Assess patient needs, plan, evaluate, and modify care to meet therapy goals.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.

• Graduate from an accredited school.
• Current state license as a Registered Physical Therapist / PT or actively pursuing licensure.
• Current CPR certification.
• Experience in plan-of-care development and skilled PT service delivery.
