Occupational Therapist (OT) - SNF ​/ LTC

A great Skilled Nursing Facility (SNF) / Long Term Care (LTC) is looking for an experienced or newly graduated Occupational Therapist / OT to join their growing team.

• Provide direct patient care to assess their medical condition, functional capabilities, limitations, and restrictions, and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for patients.
• Document and record the patient's condition.
• Educate patients and families in an appropriate occupational therapy method.
• Provide skilled occupational therapy services / interventions in accordance with physician orders.
• Assess patient needs, plan for, evaluate and modify care to meet goals of occupational therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.

• Graduated from an accredited school.
• Current state license as an Occupational Therapist / OT or proactively in the process of obtaining a state license.
• Current CPR certification.
• Experience in providing occupational therapy services in a skilled nursing or long term care setting (experience or newly graduated).
