Caregiver, Home Health Aide, Home Care

In this role, you will support individuals of all ages and abilities with daily living activities, personal care, and community engagement, promoting independence and dignity. You'll follow personalized care plans, ensure safety, and document services provided. Responsibilities

• Provide personal care assistance including bathing, dressing, grooming, toileting, and hygiene while maintaining dignity and respect.
• Assist with safe mobility, transfers, and positioning, using proper body mechanics and equipment as needed.
• Prepare meals and assist with feeding, following dietary guidelines and individual preferences.
• Support individuals with household tasks such as light housekeeping, laundry, and maintaining a safe, clean environment.
• Follow individualized care plans and behavior support strategies as directed by supervisors and clinical team members.
• Observe, document, and report changes in health, behavior, or routine to appropriate staff or family members in a timely manner.
• Accompany individuals to medical appointments, activities, or community outings, promoting engagement and inclusion.
• Provide companionship, emotional support, and encouragement to help individuals build skills and remain as independent as possible.
• Follow all health, safety, and infection control procedures, including proper use of personal protective equipment and hand hygiene.
• Participate in required training and ongoing professional development to enhance caregiving skills and knowledge.
