Location: Nevada
Center Well Home Health is seeking a dedicated Home Health Physical Therapist Assistant to deliver in-home rehabilitation, focusing on regain of strength, mobility, and independence. You will work under the Physical Therapist to implement plans and document progress with a patient-centered approach.
The role requires licensure, a valid driver’s license, CPR, and strong teamwork within a geriatric care setting. A Driver safety mindset and TB screening are part of the Humana program inclusion.
