Home Provider​/Shared Provider

Mosaic Contractors seeks a Home Provider / Shared Living Provider in Iowa to welcome an adult needing support into a stable, caring home. In this role, you'll offer day-to-day assistance with personal care, meals, transportation, and household routines while promoting independence and community inclusion. You'll follow care plans, maintain documentation, and collaborate with agency staff and families. Ideal providers are dependable, organized, and compassionate, with a safe, clean home and willingness to complete required training and home inspections.

Responsibilities

• Provide safe, stable shared living support within your home for an adult in need of daily assistance
• Assist with personal care, meals, medication reminders, and household tasks as needed
• Support transportation to appointments, community activities, and errands
• Monitor safety, promptly report concerns, and follow care and incident protocols
• Document daily activities, progress notes, and any changes in condition
• Coordinate with Mosaic Contractors staff, case managers, and families on care plans
• Promote independence, life skills, and community inclusion for the individual
• Maintain a clean, compliant, and welcoming home environment
• Follow all state regulations, company policies, and confidentiality requirements
• Participate in required training, home inspections, and periodic care reviews
