Position Description

We are seeking a compassionate and motivated Physical Therapist (PT) for a 13-week contract assignment in a Skilled Nursing Facility (SNF) located in Bloomfield, IA. The PT will evaluate and treat patients to improve mobility, strength, balance, and overall functional independence while collaborating with an interdisciplinary care team to deliver high-quality patient outcomes.

Requirements

• Active Iowa Physical Therapist (PT) license or ability to obtain prior to start
• Graduate of an accredited Physical Therapy program
• Current BLS certification (if required by facility)
• SNF or post-acute experience preferred
• Strong communication and patient care skills
• Ability to work independently and as part of a multidisciplinary team
