• Provides outpatient counseling and evidence based mental health counseling interventions.
• Documents services in EHR for billing submission in compliance with standards as provided by the clinic business office.
• Completes comprehensive social histories including patient’s mental health status, preexisting health, mental health and substance abuse problems, strengths and deficits, social role functioning, employment status, relevant cultural and religious factors, social, behavioral, environmental, and financial issues to develop a clinical assessment and treatment plan consistent with the scope of practice of a Licensed Independent Social Worker as defined by the Iowa Social Work Licensure Act.
• Collaborates with psychiatrist in caring for Behavioral health patients providing assistance and support as needed.
• Serves as a resource to the staff, providing guidance and direction for social services and mental health related issues and facilitates.
• Participates in and presents in-services and continuing education activities as appropriate.
• Participates at meetings and committees as assigned by manager and additionally meetings sponsored by agencies or groups of agencies to increase knowledge of resources and community networking.

• Coordinates care with outside sources including Lab, Diagnostic Imaging, Private Health Care Providers, Social Workers, and Public Health.
