The Community Assistant (Direct Support Professional) is either a full‑time or a part‑time position and is an active member of a team providing services to individuals with disabilities who reside in the community. Full Circle Services, Inc strives to create a fun and enjoyable atmosphere to encourage client participation in programs. Full Circle Services, Inc ensures the well‑being of Consumers and promotes their development to a more independent lifestyle.
Education- High school diploma or GED or enrolled in a high school and over age 16
- Ability to maintain yearly training (i.e., Mandatory Reporter, HIPAA, OSHA, etc.)
- No experience needed.
- Preferred experience working with persons with disabilities and/or diverse cultures.
NOTE:
The job duties listed are typical examples of the work performed by Community Assistants. Not all duties assigned to every position are included, nor is it expected that all positions will be assigned to every duty.
- Provides direct services to consumers, working nights, weekends, holidays, etc., and aids them in their homes/program and in the community to function more safely and independently, while serving as a positive role model.
- Monitors consumers’ well‑being, assists with daily life skills and behavior management training, supports individualized goals from the Individual Support Plan, and helps attend appointments when necessary, including providing transportation.
- Implements Individual Support Plans consistently according to planned strategies.
- Documents HCBS 20 Outcomes each shift of work.
- Maintains accurate documentation and billing records and/or verbal reports regarding consumers’ daily activities and progress; ensures all required documentation is complete and accurate, and submits timesheets to supervisor on time.
- Participates in the consumer’s interdisciplinary team meeting.
- Recognizes potential emergency situations, analyzes them accurately, and develops strategies to deal with such situations.
- Follows the work schedule each day (100% of time) with changes only upon supervisory approval.
- Adheres to work rules and ethics.
- Reports consumer problems and complaints in a timely manner as outlined by supervisor and company policies.
- Communicates effectively with consumers, family members, guardians, co‑workers, and supervisors.
- Maintains positive relationships with other agencies.
- Always maintains confidentiality, releasing information only to persons to whom FCS Inc. has obtained releases.
- Answers phone and/or in‑person inquiries and directs them to appropriate staff members if required; responds to routine queries about the organization and services if assigned.
- Be cognizant of consumer preferences, interests, needs, dreams, and strengths, ensuring consumer rights are protected.
- Completes General Event Reports (GER) within 24 hours of incident and immediately contacts supervisor or on‑call person.
- Attends monthly staff meetings/trainings or initiates communication with supervisor for follow‑up.
- Identifies, corrects (if applicable), and reports any safety concerns and crisis situations to appropriate personnel; follows all safety procedures and attends safety training as assigned.
- Is proactive in diffusing situations by recognizing potential crisis situations and developing strategies, informing supervisors and managers when incidents arise.
- Operates office equipment such as personal computers, photocopiers, facsimile machines, and printers, and reports or arranges for equipment repairs.
- Performs related work as assigned.
