Certified Occupational Therapy Assistant – COTA, Long Term Care

• Assist patients/residents/clients with therapy activities and exercises scheduled in a treatment plan under the supervision of an occupational therapist
• Provide, monitor, and record therapy services for patients/residents/clients with physical, cognitive, developmental, and psychosocial impairments
• Select, implement, and modify therapeutic activities and interventions consistent with competency levels, patient goals, and practice-setting or state-licensure requirements
• Exchange information with and provide documentation to the occupational therapist about patient responses and communications during intervention
• Provide information and documentation related to occupational therapy outcome achievement
• Follow accepted clinical practice guidelines of the professional organization
• Perform other related duties as requested
• Depending on location and facility needs, work in post-acute, long-term care, and home health care environments

• Appropriate education level required in accordance with state licensure
• Must be licensed as an Occupational Therapy Assistant in the state(s) of practice
• When working at a Sanford Health Facility (not required but preferred for GSS Facilities):
  Must also be certified as an Occupational Therapy Assistant through the National Board for Certification in Occupational Therapy (NBCOT)
• Conforms to the supervision and licensing standards of the state(s) where services are provided
• When applicable, may require valid driver's license
• Basic Life Support (BLS) certification required within six months of employment

Demonstrates expertise in implementing and modifying therapeutic activities under the supervision of an occupational therapist, while adhering to state licensure requirements and clinical practice guidelines. Proficient in documenting patient responses and achieving occupational therapy outcomes in various care environments.
