Physical Therapist; PT; Licensed

A great Skilled Nursing Facility (SNF) / Long Term Care (LTC) is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team! This is a part time opportunity. This is a direct hire/perm opportunity.

• Conduct assessment of referred patients requiring rehabilitation services, providing direct patient care to assess medical condition, functional capabilities, limitations, restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals based on patient capacity and tolerance under direction of the Physician.
• Set realistic and achievable goals for patients, document and record patient condition, and educate patients and families in appropriate physical therapy methods.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals of physical therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.

• Graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or actively pursuing licensure.
• Current CPR certification.
• Experience assessing and developing treatment plans for patients requiring rehabilitation.
