Assertive Community Treatment; ACT) Case Manager

We serve individuals with Intellectual/Developmental Disabilities (I/DD) , Serious Persistent Mental Illness (SPMI) , and Serious Emotional Disturbance (SED) —and we’re looking for dedicated professionals to join our team.

Full-Time, On-Site Salary: $ $21.73 – $22.61

As a member of our Assertive Community Treatment (ACT) team , you’ll help individuals with severe and persistent mental illness manage symptoms, integrate into the community, and achieve the highest possible quality of life. This role follows the ACT fidelity model and Wellvance policies.

One year of experience in the mental health field

Certification as QMHP, QIDP, CMHP , or limited/full Michigan licensure in your field

Ability to work a flexible schedule and travel within our service area Valid Michigan driver’s license , vehicle insurance, and good driving recordU.S. citizenship or legal authorization to work in the U.S.Pre-employment checks include criminal (state & federal fingerprinting), driving, education, and employment verification.
