Overview
Immanuel Pathways PACE is seeking a Primary Nurse Case Manager for its Central Iowa location. This full-time position offers a Monday to Friday schedule from 8:00 am to 4:30 pm, with pay based on experience and qualifications. Responsibilities include assessing participants’ health needs, coordinating care with the Interdisciplinary Team (IDT), and providing skilled nursing care in various settings. Candidates must hold a current RN license and have at least one year of experience with the elderly population.
