Nurse Practitioner​/Physician Assistant able to conduct home visits in Clar

The JSA program is physician‑led and advanced practice clinician‑driven, intended to care for and support Alignment Health patients with complex and chronic care needs. The role works within an interdisciplinary team environment that includes physicians, nurse practitioners, physician assistants, case managers, and other health‑care team members to ensure proper delivery of clinical and home‑based patient care.

• Conduct in‑home assessments (with some outpatient clinical work) on health plan members.
• Perform comprehensive history, physical exam, medication review, and appropriate cognitive/depression/safety screenings.
• Engage in end‑of‑life care discussions.
• Collect laboratory specimens in the home setting when appropriate.
• Identify diagnoses for active medical management and chronic disease management care plans.
• Communicate assessment findings to the PCP to address potential gaps in care.
• Provide patient/family/caretaker education with emphasis on close monitoring and follow‑up.
• Utilize knowledge of community resources for referral.
• Comply with HIPAA and maintain security of protected health information (PHI).

• Experience: Required:
  None.
  
  Preferred:
  One (1) year of prior clinical or home care experience.;
  Preferred:
  Previous EMR experience;
  Experience in care of older adult (geriatric) patients.
• Education: Required:
  Master’s degree from an accredited NP Program or PA program.

• Licensure:
  • Required:
    
    Active Nevada state Nurse Practitioner or Physician Assistant license.
  • Active RN license and furnishing number (upon hire).
  • NP only:
    Active Nurse Practitioner Board Certification / Physician Assistant Certification.
  • NPI Number, DEA, Valid BLS.
  • Valid Nevada driver license and current automobile insurance.
• Physical Functions: Regularly required to talk or hear; stand, walk, sit, use hand to finger, handle or feel objects, tools, or controls; reach with hands and arms; frequently lift or move up to 20 pounds; specific vision abilities: close vision and ability to adjust focus.
