- American Heart Association BLS certification required for all direct patient care personnel
- PALS certification required for RNs in ED (Emergency Nurse Pediatric Course (ENPC) certificate also acceptable)
- ACLS certification required for RNs in ED, CCU, PACU, MICU, SICU
- TNCC certification required
- License/Certificate verification must be dated within 30 days of start
- Fit Test required
- NIHSS certification required
- Trinity Specialty Specific Competency Testing must be completed within a year
- COVID-19 Vaccine proof or declination/exemption required
- MMR vaccination proof or IgG titer required
- Hepatitis B vaccination series required
- 2 Step PPD OR IGRA (Quantiferon/Tspot) required
- Varicella vaccination proof or IgG titer required
- Physical/Health Statement from exam within last year required
- Trinity Code of Conduct Form required
- Trinity I9 form required
- Trinity Confidentiality of Patient & Proprietary Information and Information System Security Agreement required
- Trinity Integrity and Audit Services Form required
- Trinity Non Employee Upload Form required
- Trinity Profile (Resume, Skills Checklist within a year, 2 Professional References) required
- Trinity Conflict Of Interest Disclosure Form required
- Trinity Criminal Background Check within 30 days required
- Trinity Education Verification required
- Trinity EPLS report within 30 days required
- Trinity GSA/OIG report within 30 days required
- Trinity Joint Commission Testing (CORE Exam within a year) required
