Overview
Family First Homecare provides exceptional in-home nursing care to pediatric patients with life-long health conditions. We value telling the truth, owning the details, and raising the bar, and provide support to help you deliver one-on-one care in the home. Shifts available: PRN, weekends 8:00am – 6:00pm. Start of Care:
Immediate. Clinical cares include, but not limited to: tube feedings, medication management.
- Provide care that is within the scope of licensure/certification.
- Maintain an updated clinical record for each patient and meet documentation deadlines.
- Evaluate the effectiveness of nursing services to the patient and family; report changes to the nurse supervisor.
- Consult with physicians regarding changes to patient care plans; document changes accordingly.
- Mentor and/or coach new employees during patient-specific orientations.
- Educate patients and caregivers on topics related to patient care needs.
- Participate in the Quality Assurance and Performance Improvement program (QAPI) as requested.
- Continuously seek to improve nursing care skills; attend mandatory training and in-services.
- Submit required personnel file documentation in a timely manner.
- Adhere to FFHC compliance and ethics expectations; maintain patient and family confidentiality.
- Maintain professional, supportive, and responsive interpersonal communication.
- Perform other duties as assigned.
- LPN/LVN Nursing Degree and an unencumbered LPN/LVN license in the state of employment, or multi-state license through the Enhanced Nurse Licensure Compact where recognized.
- 1 year of demonstrated experience.
- Valid driver license with proof of insurance in applicable states.
- Clear background screening.
- Working knowledge of Nurse Practice Acts.
- Valid and current CPR card/certificate with in-person instructor-led skills check.
- Clear Hep B and TB screening in applicable states.
- Ability to read and communicate in English (verbal and written).

Work is performed in the patients home and may involve lifting up to 50 pounds without an assisted device. Training may occur in an office setting.
