Hospice Registered Nurse; RN

Clarion Forest VNA is seeking a dedicated and compassionate full‑time Registered Nurse (RN) who desires a more family‑friendly, flexible schedule to join our Hospice team. This role involves overseeing a primary caseload of hospice patients and providing skilled, compassionate care that supports both patients and their caregivers. You will work closely with an interdisciplinary team to ensure comprehensive care is delivered with dignity and respect.

• Conduct thorough initial and ongoing assessments of patients and families
• Provide symptom management and end‑of‑life care.
• Collaborate with physicians and team members.
• Manage and document patient care thoroughly and accurately.
• Maintain a professional and empathetic approach to patient and family interactions.
• Participate in Interdisciplinary team meetings and on‑call rotation

• Graduate of an accredited nursing program; BSN preferred.
• Active RN license in Pennsylvania.
• At least one year of general nursing experience;
  Home Health, medical/surgical, or critical care experience preferred.
• Valid PA driver’s license and reliable, insured transportation.
• CPR certification.
• Strong communication, organizational, and critical thinking skills.

• Ability to perform tasks such as walking, bending, and lifting up to 50 lbs.
• Comfortable working in various home environments and settings.
• Visual and auditory acuity sufficient for written and verbal communication.

Hours are primarily Monday‑Friday 8:00AM‑4:00PM plus on average 1 weekend/month, 5 evenings/month as‑needed, and 2 Holidays/year. Evening hours are scheduled visits between 4:00PM – 9:00PM. Weekend Oncall hours are on a rotating basis. Weekend on‑call rotations and Evening visits would be discussed more in depth at the interview.

Starting Salary Range: $54,000 to $70,000 annually with a generous bonus system in place. Salary commensurate with education and experience.
