Our dedicated, patient-focused Home Health agency is seeking a Home Health RN Nurse Office Case Manager to join our growing team! You will be responsible for the assessment, diagnosis, and treatment of assigned patients while collaborating with our patient facing field Nurses.

• Collaborate with Field Nurses to treat, and diagnose patients
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating hospitals, physicians or specialists regarding patient care
• Maintain accurate patient medical records
