• Friday and Saturday 11pm to 7am (times can be flexed if needed) and Saturday and Sundays 7am to 3pm (times also flexible if needed)

Tobyhanna, PA - LPN for g-tube care, PART-TIME

Tobyhanna, PA - LPN for trach/vent care, PART-TIME

Lake Ariel, PA - LPN Weekends Saturday and Sunday 7am-11pm Shifts can be split has 18 hours a day on both days / Monday , Thursday and Fridays 6pm-6am

• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members
