Hiawatha Care Center is actively hiring a Licensed Practical Nurse (LPN) to provide both clinical leadership and hands-on skilled nursing care. The LPN plays a critical role in resident assessment, care planning, and day-to-day clinical decision-making. As a Licensed Practical Nurse (LPN) at Hiawatha Care Center, you will collaborate closely with nursing staff, providers, residents, families, and interdisciplinary teams to ensure high-quality resident outcomes.
What We Offer
Compensation : From $38 per hour
Additional Pay
Eligible for an additional $5 per hour worked on Saturday and Sunday (Weekend Bonus)

Key Responsibilities
Provide skilled nursing care and clinical oversight as an LPN
Assess resident conditions and respond appropriately to changes in health status
Develop, implement, and evaluate individualized care plans
Administer medications and treatments in accordance with physician orders
Communicate with residents, family members, providers, and interdisciplinary team members
Order medications and coordinate resident healthcare needs
Oversee resident admissions, discharges, and transfers
Coordinate resident appointments and transportation arrangements as needed
Supervise, support, and mentor Certified Nursing Assistants (CNAs)
Ensure documentation is accurate, timely, and compliant with regulations
Support infection control, quality assurance, and resident safety initiatives
Promote a positive team culture through leadership, accountability, and collaboration
Qualifications
Current Iowa Licensed Practical Nurse (LPN) license
Current BLS/CPR Certification
Previous LPN experience preferred
Strong clinical assessment and critical thinking skills
Ability to effectively supervise and support nursing staff
Excellent communication and leadership abilities
Ability to adapt to changing resident needs and priorities
LPN candidates must have completed or be willing to complete the Long-Term Care Supervisors Course, if required by state regulations
