LPN Pediatric Home Care

Role:

Home and Community Based Services Licensed Practical Nurse (LPN)

• Provide direct nursing care to acutely and chronically ill patients in their homes, following physician’s plan of care.
• Record thorough clinical notes and coordinate patient care activities to ensure high-quality service.
• Participate in care coordination, communicate effectively with patients, families, and healthcare team members, and report changes in patient condition.
• Adhere to infection control procedures and document visits accurately in electronic medical records.

• Current and unrestricted LPN licensure and CPR certification.
• Valid driver’s license, reliable transportation, or access to public transit.
• Ability to work independently in diverse home environments.
• Preferred:
  Home care experience, strong communication and organizational skills, and flexibility.
