Primary Objectives

Performs complex work planning, organizing, and administering services to County applicants for social service programs by interviewing applicants based on financial status, resources, and assessment of barriers preventing self-sufficiency; provides ongoing intensive case management to individuals participating in long-term programs; assigned most difficult and complex cases; performs related tasks as apparent or assigned.

This is a safety sensitive position and is subject to random and pre-employment drug and alcohol screening.

  • Identifies applicant’s needs for social services through interviews to obtain information and determine eligibility; explains rules and regulations governing eligibility and grants; evaluates personal and financial data; maintains all aspects of confidentiality; assigns cases to staff as appropriate.
  • Oversees case management services to ensure development of goal planning and empowerment of individuals and family; with a focus of successful life planning and future outcomes.
  • Reviews and evaluates program services and activities to ensure program integrity and adherence to County policies and procedures.
  • Provides advice to management on matters concerning social services activities, and furnishes technical feedback on assignments and cases, and related concerns.
  • Engages applicants and recipients in open dialogue to clearly explain rules and regulations governing eligibility, methods of payment, and legal rights.
  • Initiates procedures to approve, modify, deny, or terminate client eligibility for social service programs and grant funding.
  • Develops and maintains working relationships with clients via group interactions, one-on-one appointments, phone and, email. Creates content and posts to social media for the purposes networking, education, job referrals, announcements and community outreach. Creates/develops information brochures, program materials and other media for the purpose of informing the public of services, workshops and educational opportunities.
  • Utilizes person-centered, holistic and evidenced based approach to case management by assisting clients in completing applications; provides factual information regarding various program regulations and procedures; identifies need for other services, and makes referrals to various agencies and community resources available; acts as an advocate between other social service agencies.
  • Creates and maintains case files; maintains accurate logs and compiles records of work performed; prepares regular and special reports as required including bi-weekly client diagnostic reports through database.
  • Creates curriculum for, and facilitates, a variety of workshops, seminars, networking groups, new client and new business orientations, and other stakeholder meetings and work groups.
