ABOUT

THE ROLE

In this position you will assist individuals with developing a self-directed budget and the day-to-day management of self-directed services. Aa a Support Broker you are providing support and training to individuals and their families regarding the on-going decisions and tasks associated with self-direction. The extent of assistance provided is determined by the individual and is specified in the broker agreement signed by the individual and the Support Broker.

You will provide broker services to a caseload of individuals with an approved Self Direction Budget not to exceed 60 people.

Responsibilities
  • Assist the individual to develop and maintain a Circle of Support/Planning team and assist in facilitating planning meetings.
  • Assist the individual to schedule and facilitate meetings, identify new members, assign roles/tasks to team members, and ensure the team members are chosen freely by the individual.
  • Ensure completion of initial and annual updates to Staff Action Plans and program safeguards for Self-Hired staff for individuals who have these services in their budget.
  • Assist the individual to develop and maintain a comprehensive Self-Directed Budget that is consistent with his/her Lifeplan.
  • Provide education and training to the individual and his/her planning team in implementing the Self-Direction Budget. Work with the individual, the care manager and planning team to identify and develop initial connections in the community as identified in the Lifeplan.
  • Provide assistance to the individual in reviewing expenditures and make necessary updates to the Self-Direction Budget as needed, to ensure that it meets the needs of the individual and remains current and eligible for Medicaid funding.
  • Attend the individual's Lifeplan reviews and assist the individual to review and update the Lifeplan as requested.
Qualifications
  • High School Diploma or GED with three years of experience in Human Services working with people with intellectual and developmental disabilities (Or)
  • High School Diploma or GED with one year of experience as a Support Broker
  • Valid NYS Driver's License
