Job Title

Serve as a member of the interdisciplinary team in providing assistance with activities, social, emotional and economic concerns of the resident and family, thus enabling them to achieve or maintain an optimal level of functioning.

Associate degree in Human Services, Social Work, or medical field required.

Prior experience in long term care, social services or human services required.

Must meet state certification requirements for Social Services.
