The Town of Ipswich Council on Aging is looking for a Social Services & Outreach Coordinator to join our team and help older adults navigate life's challenges, find the resources they need, stay connected to their community, and continue living independently.

This is more than a desk job.
Every day is different. One day you might help someone apply for SNAP or fuel assistance, the next you might visit a resident at home or in the hospital, connect a family with community resources, coordinate services for an older adult, or present at a local community event.

As our Social Services & Outreach Coordinator, you'll be an important resource for older adults and their families.

You'll:

• Meet with older adults in their homes, at the Senior Center, by phone, and in settings such as hospitals, rehabilitation facilities, assisted living, and long-term care facilities.
• Listen, ask questions, identify needs, and help residents find solutions.
• Connect older adults with programs, benefits, healthcare providers, community organizations, and other resources.
• Help residents navigate applications for programs such as SNAP, fuel assistance, housing assistance, property tax abatements, and other benefits.
• Advocate for residents and help coordinate services when navigating complicated systems.
• Maintain confidential client records and keep information current.
• Recognize and respond appropriately to concerns involving vulnerable or at-risk older adults.
• Help create and deliver programs that address the social, educational, and emotional needs of older adults.
• Coordinate programs such as Project Warm Home and the Pet Care Project

• Recruit, train, and support volunteers and interns.
• Build relationships with Town departments, healthcare providers, senior living communities, nonprofits, and other community partners.
• Represent the Council on Aging at community meetings, health fairs, boards, committees, and local events.
• Give presentations and spread the word about the services and resources available through the Council on Aging.
• Bring new ideas to the table and help us identify emerging needs in our senior community.

You'll work with people from all walks of life and collaborate with an amazing network of Town departments, healthcare providers, nonprofits, government agencies, and community organizations.

We're looking for someone who is compassionate, organized, resourceful, and comfortable working with people.

You should be someone who:

• Can communicate with people experiencing difficult or sensitive circumstances with patience and professionalism.
• Enjoys getting out into the community and building connections.
• Has a "let's figure this out" attitude.

Required:

• Bachelor's degree in Social Work from an accredited college or university.
• Three years of experience providing social services, case management, outreach, advocacy, or related human services, preferably working with older adults.
• Strong communication and interpersonal skills.
• Ability to maintain confidentiality and handle sensitive information appropriately.
• Ability to organize multiple cases, projects, and priorities.
• Proficiency with computers and applicable software.
• Valid Massachusetts driver's license.

Bonus points for:

• Master's degree in Social Work (MSW).
• Experience working with older adults or vulnerable populations.
