Summary:
Responsible for conducting and coordinating the development and completion of the Resident Assessment in accordance with state and federal guidelines and the policies and goals of this facility.
Environment:
Work will be performed primarily indoors at one of our long-term healthcare facilities, throughout all areas, including in resident rooms, and on carpeted and/or tiled floors. Work will be performed there routinely around other co-workers, healthcare staff, residents, and guests. Due to the nature of facility’s business, worker may be exposed to occasional slippery floors, object on floors, chemicals, sharp objects, hazardous materials and waste (including human), blood borne pathogens, and communicable diseases, as well as high-stress medical and/or life threatening situations.
Essential Duties & Responsibilities:
· Meet physical and sensory requirements stated below, and be able to work in the described environment.
· Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessments, including the implementation of CAA’s, triggers, and care plan.
· Complete assessments of residents per their individual MDS schedule.
· Implement and update comprehensive care plan.
· Document services rendered to residents for billing purposes.
· Conduct care plan meetings with interdisciplinary team.
· Conduct daily/weekly PPS meetings.
· Weekly documentation on all Medicare/Managed care residents.
· Weekly progress notes on residents on therapy case load.
· Ensure that all assessments are completed and transmitted in a timely manner
· Monitor facilities QI/QMs for accuracy.
· Obtain needed diagnosis codes to cover medications/treatments reported on MDS.
· Implement and update care plans for residents in compliance with physician’s orders.
· Implement established nursing policies and procedures, educating nursing support staff according to facility guidelines.
· Notify resident or legal representative of changes in the plan of care per residents wishes., i.e. each change or with quarterly meeting.
· Provide education/in-services as needed on accurate charting for adl’s.
· Implement and maintain physician certification sheets on Medicare/managed care residents.
· Assist with gathering medical records for prior authorization of insurance.
· Assist with denials of payment and ADR.
· Promote favorable working conditions and relationships with the administration, medical staff, consultants, other departments, residents, family members, volunteers, agencies, and any other individual or group affiliated with the facility.
· Review charting and physician’s orders. Document in resident’s chart pertinent information about significant change in resident’s condition.
· Attend/serve on various committees as assigned.
· Schedule next months assessments by the middle of previous month and provide schedule to all team members.
· Other special projects and duties, as assigned.
Job Requirements:
· Registered Nurse with current state license.
· One (1) to three (3) years related experience; MDS Coordinator preferred.
· Must have a current/active CPR certification.
· Excellent technical, assessment and documentation skills.
· Effective verbal and written English communication skills.
· Demonstrated basic to intermediate skills in Microsoft Word, Excel, Power Point and Outlook, Internet and Intranet navigation.
· Highest level of professionalism with the ability to maintain confidentiality.
· Ability to communicate at all levels of organization and work well within a team environment in support of company objectives.
· Customer service oriented with the ability to work well under pressure.
· Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity.
