A Physical Therapist provides high-quality patient care through evaluation, treatment planning, and the application of physical therapy modalities and procedures. Additionally, this role involves family training to support patient recovery. All practices are performed in compliance with the regulations governing Physical Therapy and in alignment with the philosophy and policies of City of Hope. This role will support in an outpatient setting.
Responsibilities- Screen patients to determine need for intervention and initiate referrals as appropriate
- Evaluate patients using age-specific tools and interpret data to plan treatment
- Establish goals and treatment plans based on evaluation data, rehabilitation potential, discharge plans, and coordination with the patient, family, and other care team members
- Modify treatment programs according to patient needs and progress, terminate treatment appropriately, and develop discharge and post-discharge plans
- Complete evaluations, progress notes, discharge summaries, and third-party reimbursement reports per established departmental requirements
- Assign treatments to physical therapy aides and physical therapist assistants; assess quality of care provided
- Doctor of Physical Therapy from an accredited school or Master’s equivalent
- Entry-level position; no experience required but oncology interest is preferred
- Current Physical Therapist license in the State of California or PTLA status in California
- BLS certification through the American Heart Association
- Hourly Per Diem Rate: $58.80
