Nurse Coordinator - Supportive Care; Pre&Post Care and Rehab

The Department of Supportive Care is seeking a dedicated and compassionate Nurse Coordinator (RN) to join our growing team. This role supports physicians and Advanced Practice Providers across inpatient and outpatient settings, triage, provide patient education, and deliver coordinated pre‑ and post‑follow‑up care for the Physical Medicine and Rehabilitation (PM&R) service, ensuring patients receive timely guidance and comprehensive support. The position is Monday‑Friday, 8:00 am‑5:30 pm.

• Manage and prioritize incoming referrals to ensure timely and appropriate care.
• Ensure prehabilitation protocol adherence, timely responses or escalation to providers, and monitor and manage inbox messages.
• Collaborate with the interdisciplinary team to coordinate care for patients with complex needs, partnering with physicians, APPs, and clinical teams across the OC network.
• Provide comprehensive education to patients and families, supporting informed decision‑making throughout the care journey.

• BSN
• Three to five years of experience in the field of expertise.
• Experience with care coordination pre/post procedures, and rehabilitation.
• Current California RN license.
• Basic Life Support (BLS) through the American Heart Association.

• Experience in Physical Medicine and Rehabilitation, Interventional Pain, Sports Medicine, Orthopedics, Neurology, Neurosurgery, and Palliative Care.
• Oncology background.
