- Providing nurse visits to home health patients.
- Performing an initial, comprehensive assessment and documenting everything & uploading to Agency Portal.
- Obtaining and documenting physician orders.
- In consultation with the assigned Clinical Team Manager, initiating and regularly re‑evaluating and revising the plan of care.
- Coordinating care with the assigned Clinical Team Manager, physician and other home care team members, informing them of significant changes in the patient’s condition and needs.
- Performing re‑certifications, resumption of care, transfers, and discharges as requested by the assigned Clinical Team Manager and completing and submitting all related documentation.
- Assessing the need for services of other team members (PT, OT, ST, MSW, HHA).
- Performing Home Health Aide supervisory visits per regulatory requirements.
- Demonstrating knowledge of regulatory requirements.
Patients need care 7 days a week. You should be able to work at least two to three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer).
Qualifications- Visiting RN for Home Health: 1 year required.
- Registered Nurse: 1 year required.
- Current CA RN License.
- BLS from the American Heart Association and/or the American Red Cross.
- NSO Nursing License Insurance.
- IV/Phlebotomy Certified.
