Essential Responsibilities
  • RN First Assistant renders direct patient care as part of the perioperative role by assisting the surgeon in the surgical treatment of the patient.
  • The RNFA must have documented knowledge and skills acquired after specialized preparation and formal instruction.
  • • Safety of the patient is the primary consideration in selecting a first assistant. When no qualified physician is available, the registered nurse who possesses the appropriate knowledge and technical skills is the best non‑physician to serve as first assistant.
  • • The RNFA practices under the direct supervision of the surgeon during the surgical intervention.
  • • The RNFA must perform only as first assistant and not concurrently as scrub nurse.
  • • Only in extreme emergencies should an RNFA be expected to assist on procedures that present an unusual hazard to life.
  • • The RNFA must adhere to institution policies and remain within the scope of practice as stated by the Nursing Practice Act of the State of California.
  • • Technical functions: positioning, prepping, draping of the patient; providing retraction, managing instruments; providing hemostasis; performing knot tying; providing closure of layers; assisting surgeon at completion of the procedure; and affixing and stabilizing drains, cleaning the wound, and applying dressings or casts.
Standardized Procedure:
Intraoperative Retracting
  • Retract tissues or organs by hand, closely observing the operative field at all times.
  • Place or hold surgical retractors in the position set by the surgeon with regard to surrounding tissue.
  • Packing sponges into body cavities to hold tissue or organs out of the operative field.
  • Manage all instruments to prevent obstruction of the surgeon’s view.
  • Standardized Procedure:
    Intraoperative Hemostasis
  • Aspiration of blood and other fluids from the operative site using suctioning techniques.
  • Sponging the wound and utilizing pressure as directed.
  • Placing hemostats on other instruments to clamp tissue or bleeding vessels.
  • Applying electrocautery tip to clamps or vessels as directed.
  • Placing suture ligatures on vessels or tissue as directed.
  • Performing knot tying firmly to avoid slipping.
  • Standardized Procedure:
    Intraoperative Tissue Manipulation
  • Expose and retract tissue.
  • Clamp and sever tissue.
  • Grasp and fixate tissue with screws, staples, and other devices.
  • Drill, ream, and modify tissue.
  • Cauterize and approximate tissue.
  • Procedure in Event of Surgeon Incapacitated
  • Maintain hemostasis according to the approved standardized procedure.
  • Keep the surgical site moistened as necessary.
  • Maintain integrity of the sterile field.
  • Remain scrubbed in appropriate attire (gown, mask, gloves, cap).
  • Remain at the field until a replacement surgeon is located.
  • Requirements for RNFA in Expanded Role
    • Meet all requirements of the hospital Non‑physician/Non‑Employee Policy.
    • Certified in basic Cardiopulmonary Life Support (BLS).
    • Nationally certified operating room nurse through the Association of Operating Room Nurses (AORN).
    • Minimum three years of operating room experience in both scrub and circulating roles.
    • Proof of successful completion of a structured RNFA course and 20 hours or 10 cases proctoring by the sponsoring surgeon.
    • Approval from the surgical sub‑specialty of the sponsoring physician.
    • Annual evaluation by hospital staff and sponsoring surgeon for compliance to operating room policies.
    Basic Qualifications
    • 3‑year recent (within the last 5 years) full‑time operating room experience, including both scrub and circulating roles.
    • Successful completion of a structured RNFA course and 20 hours or 10 cases proctoring by the sponsoring surgeon.
    • Registered Nurse Operating Room Certificate from Association of Perioperative Registered Nurses or Registered Nurse First Assistant Certificate.
    • Registered Nurse License (California).
    • Basic Life Support certification.
    • Advanced Cardiac Life Support certification.
