The Clinical Supervisor, Home Health Palliative Program is responsible for coordinating, supervising, and evaluating high-quality home health services for chronically ill patients with serious, progressive, or complex conditions. This role ensures that care is patient-centered, clinically appropriate, interdisciplinary, and focused on symptom management, functional support, advance care planning, caregiver education, and prevention of avoidable acute care utilization. The Clinical Supervisor provides day-to-day clinical leadership to assigned personnel and supports compliance with applicable home health regulations, standards of practice, documentation requirements, and organizational policies.

Job Responsibilities
  • Coordinate and oversee care for palliative patients with chronic and advanced illnesses throughout the home health continuum.
  • Review referrals and assign clinical staff based on patient needs, acuity, and goals of care.
  • Lead interdisciplinary collaboration to promote symptom management, patient comfort, and quality of life.
  • Provide clinical guidance and support to field staff on palliative care best practices and care coordination.
  • Monitor patient outcomes, identify risks, and implement proactive interventions to prevent avoidable hospitalizations.
  • Conduct clinical record reviews and documentation audits to ensure regulatory compliance and quality standards.
  • Facilitate transitions of care, including community resources, hospice referrals, and discharge planning.
  • Partner with physicians, clinicians, caregivers, and community providers to deliver patient centered care.
  • Support program growth, quality improvement initiatives, and palliative care education for staff and referral partners.
  • Mentor and educate clinical team members on chronic disease management, symptom recognition, and interdisciplinary care delivery.
Qualifications
  • Current Registered Nurse (RN) license in the State of California.
  • Graduate of an accredited nursing program; BSN preferred.
  • Minimum two (2) years of palliative care and/or home health nursing experience required.
  • Supervisory or management experience in home health, hospice, palliative care, or related healthcare setting preferred.
  • Palliative care certification and/or program management experience preferred. Position Details
