Riverside Health Care Facilities Inc. is currently seeking a motivated and compassionate Registered Practical Nurse (RPN) to join our Community Nursing team. The successful candidate will provide high-quality, client-centered nursing care while supporting individuals across the continuum of care in community and acute care settings.

• Graduate of an approved Registered Practical Nursing or Registered Nursing Program.
• Current registration in good standing with the College of Nurses of Ontario (CNO).
• Current Basic Cardiac Life Support (BCLS)/Cardiopulmonary Resuscitation (CPR) certification.
• Valid Ontario Driver’s License and access to a reliable personal vehicle.
• Demonstrated strong nursing knowledge, clinical skills, and sound professional judgment.
• Excellent interpersonal and communication skills, including effective verbal and written communication.
• Demonstrated critical thinking, leadership, and mentoring abilities.
• Proficiency with computer systems, electronic documentation, and healthcare applications.
• Commitment to practicing in accordance with Riverside Health Care’s Code of Conduct.
• Ability and willingness to work flexible hours, including days, evenings, nights, weekends, and statutory holidays as required.
• Knowledge of and compliance with Occupational Health and Safety legislation, policies, procedures, and worker responsibilities.
• Strong interpersonal skills with the ability to work independently and collaboratively with interdisciplinary teams, front‑line staff, clients, families, and management.
• Demonstrated ability to function effectively in a fast‑paced healthcare environment with excellent organizational, prioritization, and time management skills.
• Dependable with a satisfactory attendance record.
• Proven ability to perform the essential duties of the position on a regular basis.

• Practice in accordance with the Standards of Practice established by the College of Nurses of Ontario and Riverside Health Care policies, procedures, and standards.
• Accept clinical responsibility and professional accountability for the nursing care provided to assigned clients.
• Work collaboratively within an interdisciplinary team to provide care across the continuum of services, including community and acute care settings, by:
• Coordinating and implementing evidence‑informed clinical interventions;
• Evaluating client outcomes;
• Facilitating care transitions;
• Supporting ongoing care planning;
• Collaborating with clients, families, caregivers, and healthcare partners.
• Provide individualized nursing care and support services that promote client health, safety, independence, and well‑being.
• Assess, monitor, document, and report client conditions and changes in accordance with professional and organizational standards.
• Adhere to the Ethical Framework for Nurses in Ontario as established by the College of Nurses of Ontario.
• Maintain client confidentiality and uphold professional, ethical, and legal standards at all times.
• Contribute to a safe, respectful, and positive work environment.

• This position may be required to work days, evenings, nights, and weekends

Position Rate: $34.62 $ 36.35 $ 38.16
