Dignity Health Mercy San Juan Medical Center is a 370‑bed not‑for‑profit Level 2 Trauma Center located in Carmichael, California, serving north Sacramento County and south Placer County for over 50 years. • Perform social work assessments and interventions for inpatients, outpatients, and emergency department patients.
• Provide patient‑ and family‑centered care that values self‑determination and compassionate ethical problem‑solving.
• Perform comprehensive assessments and develop treatment plans that integrate medical, social, and resource issues.
• Collaborate with all healthcare team members to develop and implement patient‑centered plan of care.

• At least two years of clinical or medical social work experience.
• Master’s degree from a school of social work accredited by the Council of Social Work Education.
• Licensed Clinical Social Worker in California.
• Excellent customer service and presentation skills.
• Strong interpersonal and written communication skills.
• Strong analytical and problem‑solving abilities.
• Ability to manage multiple tasks or projects effectively.
• Ability to work independently with high degree of attention to detail.
• Ability to work efficiently in a fast‑paced environment with changing priorities.

$57.07 - $64.57 per hour
