Description- Conduct thorough evaluations of patients in both inpatient and outpatient settings, and craft tailored treatment plans to meet individual needs.
- Implement physical therapy interventions emphasizing mobility, strength, balance, and functional improvements.
- Collaborate effectively with nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes.
- Provide education to patients and their families regarding therapeutic objectives, transition planning, and home exercise regimens.
- Maintain detailed documentation of evaluations, treatments, and progress, ensuring compliance with facility and regulatory standards.
- Engage in interdisciplinary team rounds and conferences to guarantee cohesive patient care.
- Doctorate in Physical Therapy (DPT) or an equivalent accredited degree.
- State licensure as a Physical Therapist
. - CPR certification desired.
