Landmark Infusion is seeking a Clinical Dietitian to join our team! In this role, you'll work closely with a multidisciplinary care team to deliver high-quality, personalized nutrition support to patients managing complex medical needs at home.
This is a hybrid role: candidates must live in the Dallas–Fort Worth area and be able to commute to our office in Irving, Texas full-time, Monday through Friday. We're looking for someone with excellent time management, strong communication skills, and the ability to collaborate closely with pharmacists, physicians, and clinical staff.
Initial Dietitian Assessments- Receive referrals from the TPN pharmacist when a new TPN patient is admitted to Landmark service
- Review clinical documentation and conduct a comprehensive nutrition assessment by phone, performing medical nutrition therapy and making individualized recommendations for new TPN patients within 4 days of hospital discharge
- Document the initial assessment in the medical record within 3 days of the assessment
- Note:
assessment and documentation timelines are dependent on Registered Dietitian availability (vacation, scheduled days off, and weekends included)
- Communicate and work daily with Landmark staff
- Discuss lab results, weight changes, weaning protocols, and diet advancement with the care team
- Provide verbal nutrition recommendations to pharmacists
- Share updates on new nutritional guidelines and best practices
- Summarize weekly TPN pharmacist/dietitian communications in an email to the working group, referencing patients and topics discussed
- Support additional tasks as delegated by Landmark staff, such as monthly physician reports, weight chart documentation, and referral calls
- Follow up with patients as directed by the TPN pharmacist
- Review clinical documentation as needed and conduct follow-up nutrition assessments by phone, performing medical nutrition therapy and making individualized recommendations for existing TPN patients
- Document follow-up notes in the medical record within 3 business days of the TPN pharmacist's consult request (or per TPN team timelines)
- Communicate with referral sources — including dietitians, physicians, and mid-level practitioners — as needed to gather follow-up information
- Bachelor's degree in Nutrition, Dietetics, Food Science, or a related field (Master's preferred)
- Registered Dietitian (RD/RDN), licensed in Texas
- Completion of an ACEND-accredited dietetic internship
- CNSC (Certified Nutrition Support Clinician) preferred
- 1–3+ years of clinical dietetics experience
- TPN experience required
- Strong communication skills for phone-based/telehealth patient counseling (much of home infusion dietitian work is remote)
- Ability to work independently and manage a caseload
- Detail-oriented documentation practices for compliance and audit purposes
- Comfortable working with EMR/EHR systems and infusion-specific software
