We are seeking a dedicated Licensed Vocational Nurse Care Coordinator to join our team onsite in Irving, TX. This temporary-to-permanent role offers a unique chance to work closely with patients, advising them on Medicare practices and collaborating with healthcare teams to optimize patient health and wellness.

In this role, you will:

• Manage a case load of 10, patients with identified gaps in care in collaboration with the Health Care Team.
• Work proactively to coordinate preventative and follow-up care for patients receiving care from participating providers.
• Review patient medical records to identify care gaps and schedule needed preventative services using evidence-based protocols.
• Foster a team approach by collaborating with patients, families, primary care providers, and other healthcare team members.
• Assist Nurse Navigators in managing medical stability in patients with high-risk disease management and complex comorbid conditions.
• Must have a valid Texas or compact license with a minimum of 2 years of experience in a primary care clinic.
