Home Health Registered Nurse

Overview

Medical City Healthcare is seeking a compassionate and clinically skilled Home Health Registered Nurse to provide high-quality, patient-centered care in patients’ homes.

As a key member of the Healthcare & Medical Services team, you will deliver skilled nursing interventions, educate patients and families, and coordinate care to support safe recovery, chronic disease management, and optimal quality of life within the community setting.

Conduct comprehensive in-home assessments and develop individualized care plans.

Collaborate closely with physicians, therapists, social workers, and other interdisciplinary team members.

Perform skilled nursing tasks such as wound care, IV therapy, medication administration, monitoring vital signs and disease symptoms, and teaching patients and caregivers how to manage conditions like heart failure, COPD, diabetes, post-surgical recovery, and other complex medical needs.

Document all care accurately and promptly using electronic health records, follow established clinical protocols, maintain infection prevention practices, and promptly alert the clinical team of changes in patient status.

Prioritize and organize daily visits, travel between patient homes, and adapt to changing needs while maintaining a high standard of safety, professionalism, and compassion.

Serve as a trusted clinical resource, promote self‑management, adherence to care plans, and clear understanding of medications, treatments, and follow‑up care.

Strong critical thinking and clinical judgment skills.

Ability to work independently in varied home environments.

Excellent communication skills and flexibility.

Comfort with mobile technology for documentation and communication.
