Grace Unlimited Home Health Care is seeking experienced
PRN RN Case Managers to provide high-quality home healthcare to clients primarily in
Dallas County
, with coverage in surrounding Dallas–Fort Worth areas as needed. This role is ideal for nurses seeking
flexible, PRN work while delivering compassionate, patient-centered care in the home.

Role Responsibilities:Provide professional, compassionate nursing care in clients’ homes
Perform assessments and manage individualized care plans
Accurately document care using
OASIS/Kinnser
Manage diverse patient needs in a home health setting
Respond to PRN and urgent visit needs as assigned
Qualifications:Graduate of an accredited nursing program
Active Texas RN License
Valid Driver’s License and reliable transportation
Minimum 1 year of home health experience
Experience with
OASIS/Kinnser preferred
Ability to travel locally within
Dallas County
Flexible availability for PRN visits
Work Location:Client homes throughout
Dallas County
Field-based / on-the-road role
