Halcyon Home is hiring a Home Health LVN PRN - Per Visit to support our growing geriatric home health population in Irving/Arlington/Eastern Tarrant Country, TX and surrounding areas. We offer competitive pay and wonderful team support.

- Under the direction of the RN, assist in identifying the patient’s physical, psychosocial, and environmental needs as evidenced by documentation.
- Participate in planning and implementing care in partnership with the patient, representative (if any), caregiver(s), and the RN. Provide services that are ordered by the physician as indicated in the plan of care.
- Provide patient and caregiver education.
- Provide care using infection control measures that protect both staff and patient.
- Ensure the continuity of care through delivery of quality patient care.
- Provide effective communication to the patient, family, team members, and other healthcare professionals as evidenced by clinical notes, case conferences, and communication notes. Complete appropriate documentation in a timely manner.
- Monitor assigned cases to ensure compliance with requirements of third‑party payors.
- Participate in the Agency’s QAPI program.
- Participate in the Agency‑sponsored in‑service trainings.
- Demonstrate commitment, professional growth, and competency.
- Promote the Agency philosophy and administrative policies to ensure quality of care.
Education: Graduate of an accredited school of vocational/practical nursing.
Licensure: Active, state license as a licensed vocational/practical nurse, or a multi‑state license issued by a Nurse Licensure.
Experience: One year of experience as a licensed vocational/practical nurse in a clinical care setting required.
- Home health experience, preferred.
- Homecare Homebase platform experience is a PLUS!
Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of current CPR certification.
Transportation: Reliable transportation and valid auto liability insurance.
Payrate$35 per visit
