Role: Licensed Practical Nurse (LPN) providing in-home care for Pediatric and/or Adult patients.
Responsibilities Include- Assessing, planning, implementing, and evaluating patient care using the nursing process.
- Providing skilled nursing services based on prescribed orders.
- Monitoring signs and symptoms to identify physiological and psychosocial changes.
- Collecting and analyzing data from healthcare team members, documenting nursing diagnoses.
- Current LPN/LVN license in the practicing state.
- Valid TB or Chest X-Ray and BLS certification.
- Preferred: at least one year of professional nursing experience.
