Job Title
Shift: 5x8/DAYS (8:30A-5P: Alternating Weekends), Length: 13 weeks, GT: 40, Start: 6/15/26 or sooner
Position Requirements:
- At least 2 years of home health experience
- All candidates must have homecare homebase/OASIS experience within the last year to be considered
- Experience with Wellsky EMR and wound care would be a bonus
- Experience with on call / triage
- Reliable and dependable
- Excellent time management skills
- Professional
- Customer service excellence, compassion and caring
- Licensure in hand at time of submission
On-call Schedule and Expectations:
On-call would be one to two weeks a month from 5:30pm- 8:30 am weekdays. It entails phone triage, very rare does it require a home visit.
Productivity Requirement:
It is a 30 pt per week for full time Nurses. Our point system:
- 1 revisit= 1pt
- SOC= 2pts
- ROC = 2pts
- Recert =1.5pt
- Oasis discharge =1.5pt
