The LPN Medication Nurse provides licensed nursing support in a senior care setting, focusing on safe medication administration, health monitoring, and accurate clinical documentation.

• Administer medications and treatments following physician orders and the Six Rights of Medication Administration
• Document accurately in MAR/TAR in real time
• Monitor residents' symptoms, administer medications for relief, and report any refusals or adverse reactions
• Conduct Service & Health Updates (SHUs) and transcribe physician orders
• Observe and report physical, emotional, or cognitive changes
• Maintain medication carts, perform narcotic counts, and ensure compliance with regulations

The role emphasizes preventing medication errors, early detection of health changes, and safeguarding resident safety through clinical precision and adherence to standards.

• Active LPN license in good standing
• CPR and First Aid certifications
• Strong assessment and documentation skills
• Experience in senior care or medication administration preferred
